Actually that's not entirely true. Dr Crane, Dr Chen, and Dr Schechter all do ALT and MLD phalloplasty and all take insurance. No other surgeons in the country are doing these two techniques. Schechter also is the first to work with Medicare (Medicare will only pay 80%, you are left with 20% to pay so supplement insurance or cash for the remaining 29-35k) but in the near future I'm sure Crane and Chen will as well.

I had ALT phalloplasty with Dr Crane in September 2014 my second stage will be in June 2015. Insurance covered 100% (Kaiser of NorCal) only came out of pocket 15 bucks for a post op script that wasn't covered which we then found out was covered later lol. Quite right, that info is out of date. When I first spoke with Dr. Schechter a year or so ago, he wasn't offering ALT. He preferred RFF and MLD. But it seems like interest in MLD has declined, and now he's offering ALT as well.

Yes I think this is due to the lack of sensation reports. I've had a few mld buddies and many want sensation layin grafts from the forearm now as they did not get as much as they were initially told. This was my swaying point for years at a time after I seen Miro in 2010. But we shall see, 3 of my buddies had dates with him in the next few months I'll definitely keep you posted as I find out more.
