I've never heard of a surgeon who does a test to ensure that a patient isn't lying about quitting smoking. I think they will take you on your word.

I have heard of surgeons who have refused to operate on smokers. I don't know Dr. Medalie's policies but I would not be surprised if he reschedules surgery if a patient hasn't managed to quit smoking according to his requirements.

Best way to get a definitive answer would be to ask someone in Dr. Medalie's office.
